Health Secretary Alan Milburn is under fire after his department sent condolences for the death of a Basildon man - who is alive and well.

Mark Langford complained to Mr Milburn about the poor treatment his father, William, received in Basildon Hospital's A&E department.
But in a reply Patricia Smith, in the regional directorate of health and social care, said: "I was saddened to learn of your father's death and I would like to extend my deepest sympathies."
Mark, of Kent View Road, Basildon, said his father had made a good recovery after a brain haemorrhage.
He added: "They sent out a stock sympathy reply." The Department of Health apologised.
